I ended up emailing them to ask if the program was still active as I've yet to receive my truck yet and wanted to know what the expiration date was. I did not get a specific date but did confirm the program is active. The response i received was this:

"Thank you for inquiring about our BonusDrive program. There are two parts of eligibility for this special offer. As long as you or a member of your household are (1) with one of our participating companies or organizations and (2) you buy a new 2018-2020 car from one of our participation automotive brands. Complete an application and we'll process your request.

Cash back offer ranges between $250 and $500 depending on purchase date and vehicle selected. Qualifying vehicles means purchase or lease of a brand new FCA branded vehicle (1) by a household who owns or leases a non-FCA branded vehicle, or (2) that replaces a non-FCA branded vehicle. Proof of non-FCA branded vehicle will be required. Cash back for any purchase or lease using the "Hyundai Circle Program" shall receive $100.

If you would like to apply online, simply open this link BonusDrive. Enter all required information to match your proof of purchase and within sixty (60) days of purchasing or leasing your new never previously titled 2018-2020 vehicle.

Applied online on 30th, got update email on 7th, and got an approval email today! Just waiting on check now.
 
This Thursday I got an email saying the check was in process and would be sent via priority mail when done.
 
Mailed my form in a week ago, got the "we got it" email a few days later, got the "you're approved" email yesterday.
 
What are the "participating companies or organizations" that meet eligibility?

they aren’t listed anywhere. You can try places of employment for yourself or anyone in your household, also various credit unions and insurance companies. If none of those work, I have heard you can choose Allstate even if you do not have them for insurance you
 
Got my check today without getting tracking info or indication that the check was even mailed. I got a notice saying everything was approved then the check just showed up!
