The main difference is the built in scale. That is very nice to have. I've never used the B&W but I use the Weigh Safe True Tow heavy weight hitch. They didn't have the middle weight when I bought mine otherwise I may have bought that one. My trailer is 7400 lbs loaded, 33' long and it really controls the sway. Don't get me wrong, I do feel it when semi trucks pass me but it's not drastic at all. I love my heavy weight hitch but if the middle weight hitch works for your trailer weight, I would get that one. The heavy weight is very heavy. I used the Equalizer hitch before this and this hitch is easily as good if not better. Here's a pic of the scale on it. This is my tongue weight. And here is a pic of the app you use to calculate the distribution weight. Notice the scale weight is slightly different from the weight I entered into the app. That's because the pics are from two different camping trips. Tongue weight does vary slightly from trip to trip. When your bars are on the hitch the scale will read closer to what the distributed weight on the app says. Then you adjust the distribution to match what the app says.
