The FCW, ACC, LKAS and auto dim head lights are all controlled by the DASM in the windshield. Doesn't matter if you have them "enabled" or not, they are there. That module does it. You could probably remove it and then go in with AlfaODB and change all the settings listed in the how to on adding...
If you search, you will find this can be common issue. They redesigned the tube to fix this, but I'm not sure the year it started. It has to do with overfilling and the vent tube filling with fuel. You can also remove the tube and drain the excess fuel.
You can clear it by getting the underlining issue fixed. You could use AlfaOBD to disable the system, but you will lose everything that it does like ACC or auto dimming headlights or LKAS if equipped. Even then, not sure if it will go away if the actual problem isn't fixed.

Yes on both questions. You can get part numbers on most parts sites by putting in your vehicle info or call a parts department. Sorry, I don't have the part number, but it's easy to find.
I've had Rhino in 3 trucks and it's held up fine. My Dakota was 23 years old when I got rid of it and had it since day 1. Never an issue. I think they are all good as long as the prep is done properly.
