I have a 2019 Ram 1500 5.7 with just over 80k miles that I've had for 4-5 years and over the last few months, whenever the engine is not warm (regardless of ambient air temperature), it runs rough and sounds pretty bad for the first several seconds after being started. After 10-25 seconds, it sounds completely normal and when the engine is warm it sounds normal on startup. Video doesn't capture the sound very well, but I tried:
My first thought was maybe the oil takes a while to circulate on startup but the noise seems odd even for a poorly lubricated engine. I change the oil myself every 5000 miles with Pennzoil Ultra Platinum, so there is no lack of proper maintenance. Any thoughts on what this could be and if it's a concern? Was thinking of doing a Blackstone laboratories oil test on my next oil change too.

Either a broken stud or cracked exhaust manifold.

Will check more thoroughly soon but an initial inspection revealed the bolts were solid and the heat shield was not loose and didn't see signs of cracking.

Usually start up noise could be oil pressure related or (on hemis) the exhaust manifold or studs. It's common.

Both go away when the engine warms up.
It's more of a concern if it doesn't go away.

At 80K you are "in the zone" for exhaust issues, sadly.

Sounds just like mine and my passenger side leaks. Gonna replace it with BD when the weather warms up

My passenger side is cracked as well. Are you replacing both sides with BD or just the passenger side for now? I understand the driver’s side is more difficult to replace. I’m sure I will have to replace both at some point but I would rather just do the passenger side for now if there are no concerns surrounding BD on one side and OEM on the other. I can’t find anything to suggest this would cause any problems down the road. Any thoughts?
