Do you still have the Lane Keep assist button on your headunit and the Adaptive Cruise Control on your steering wheel. Doesn't make much sense that ACC won't work without Lane Keep Assist on.

Here's how I have mine, no issues being too close. Mess around a little bit. It's all up to your liking. I did have mine sticking out further just enough to see the AWE logo but I decided to tuck them back in.

Probably sounds terrible if you have MDS if it says not designed for active fuel management. I don't think you'll ever get away from a system that sounds exactly the same in normal mode and MDS. I had both Borla ATAK and now AWE 0fg catbacks and you can tell when it switches over.

Mine has 3.92 gears as well and the AWE system is not bad at all. It's actually quiet on the highway unless you're revving it to pass or go up hill. The MDS tone is really only annoying at low speeds so I just set the gear limiter to 8. I still like it much better than the borla atak.
MPRB is a good brand. Had it on a silverado and it sounded pretty good. I've been cautious about American Trucks because of the poor reviews. Same with CARID (First hand bad experience).
Not sure if you have air suspension but if you lift it to OR2 and remove the driver's side spare tire heat shield, it will slide right out. If you don't have air suspension, you will need ramps or jacks...if you want to remove the exhaust in piece. If you ever decide to put the factory exhaust...
If you are concerned disconnect the midpipe from the borla muffler. Then realign the midpipe to the engine side and tighten down. That way it's straight. Then bolt up the muffler to the midpipe.
