I got it off ebay.... most craigslist or others were only in sets of 4. I reprogrammed the speedo with the rough country programmer, which was pretty easy to use. I also had Costco put another tire pressure sensor in the spare in the event I want to use it in the rotation.
I have the Dualer A/T Revo 3's in a 275/65R20. They are the lightest A/T in that size (51 lbs), and seem to have similar traction ratings to a KO2 with better hydroplaning resistance. I have only had them on for a few weeks, but they ride quiet and seem to ride better than the stock AT3WA...
If it were mounted the other way, the hanger would potentially be the lowest point rather than the tire. If the hanger were to drag/break on an obstacle, then the spare falls off. The sidewall of the spare is generally more forgiving.
I mounted it the same as the factory full size spare that was in there previously. I think it is stored this way due to the way the cable clamp holds it in.
I had a similar noise that ended up being a sway bar end link that was a little loose on the driver's side front. Tightened it up and clunk went away. Was most noticeable with the steering wheel slightly off center/turning.
