I started my truck up this weekend and heard a horrible noise (even my 15 year old son heard it) Took a video of it and brough it in to my mechanic. Sounds like a tick, but not sure if it is the manifold, lifter, or something else. I have low miles on my 2019 5th gen around (37000). Mechanic today said all the bolts on the manifold are tight and he felt it might be a lifter. They are keeping it over night and the most experienced mechanic will be there tomorrow to take a look. I am freaking out right now. How the hell does a truck with 37000 miles have a bad lifter already? Obviously past my 5 year 60000 mile warranty. What do you guys think? I hope they are wrong. I asked them if they saw any cracks in the manifold and they said no. Here is a video...VID_20250421_081751471[1]

go to harbor freight and get one of these so you can pinpoint where the sound is coming from.
I am no mechanic so I have no clue what I would be hearing lol. It is at the local shop now.
 
It was on cold start. It gets less noticeable as it warms up.

Gotcha, based on a lot of videos on the subject, it's less likely to be the big lifter issue we all dread. That tends to show symptoms at full operating temperature.

Note sure if you seen this video, it's set at the main lifter tick symptom at 13 minutes 24 seconds.

Thanks for the video. Mine is worse sounding at start and for the first couple minutes after then it gets quieter after that on idle. Heard it during the first acceleration after the cold start to 25 mph right outside my house too then gets harder to hear while driving.

Hopefully it's one of the lessor issues based on that. The symptoms sound more of an exhaust manifold crack if it's true all the bolts are tight. It could be a failed lifter hydraulic portion, which he covers in the 4 min 15 sec, which matches your symptoms, not too bad if that's the issue.
 
Watched a million and one videos on the topic since last post, but did not find one that sounds like my problem. In the video posted, when I put my phone in the wheel well it sounds like to me a loose piece of metal some where vibrating on the truck....Exhaust? In the YouTube video the guy states lifters will make a ping pong ball irregular noise and usually after the truck has been warmed up at idle. Mine makes this other sound right away at start up.
 
Could be the manifold heat shield making the metal noise which means.. broken bolt. You’ll be better off jacking up the front, removing the wheel and fender liner and having a better look.

The owners manual on my 22 Big Horn (5.7 e Torque) states that it is not unusual to hear lifter tick when started and cold. Did the sound go away after it warmed up?
 
Got a call from the shop and they said it it could be a failed lifter, injector or spring in #3 or #5 or both. I am having them look under the heat shield of the manifold to make sure there is no cracks present since they said bolts were tight and wanted them to do that before they start opening valve covers. They said they used a stethoscope to diagnose it to #3 or #5. It's sounds like it is going to be one of those days....

That would point to a leak in the exhaust, specially if it quiets down after couple minutes.
The exhaust will sound like sharp escaping air sound, lifter more metallic, like a ball peen hammer tapping for a better word.
This was my passenger side broken stud.

They completely took off the heat shield and examined the manifolds. No cracking of bolt, no cracks in the manifold, no leaks. Going into the valve covers today. Mine sounds much more like banging metal than yours....Not good.
 
Update bleeding dollar dollar bills.... Saw surface wear on pushrods and lifters on 3 and 5. They owner and head tech has seen this before on those engines and advised me to get a whole new cylinder head for the left side instead of just fixing the rods and lifters. Don't have much choice but to fix it (don't want to drive it like this) and I trust this shop (most honest one I have ever been to, highly reviewed in my area). Going to fix but tempted to get rid of it. It would have to do this a year and a half outside of warranty and had quite a few issues during warranty time frame.

Sorry to hear that man, especially at just under 40,000 miles.
Want to puke right now. I have very little faith left in this vehicle. If I bring it to a dealer like this they are going to screw me over big time (well they always do just a million times worse left unfixed). Wife said might as well start looking. This should not be happening at 37000 miles.

Yup, really sorry about that man. Tough spot to be, I would consider doing the absolute minimum to fix it and trade the truck in on another vehicle if you don't plan on keeping for the long term.

I wouldn't leave someone with a ticking bomb, but do what is needed and cut your losses on that truck.

This is my first major repair outside warranty, but to start off the repairs like this is a joke to the tune of $4500. Was planning on a road trip in August about 2500 miles total but don't trust this thing (I trust the repair, but not the truck any longer). Not going to show up to a dealer unrepaired. They were going to hear it and bend me over twice as far as they normally would 😂. I might sell it myself as well and I am not the type to screw someone over. Even my 15 year old heard it and said what the heck it that.
