2 (or More) Audio inputs at once

I have had already a few times that I hear 2 (or more) songs going through the system at once.
I have the 12" Uconnect system with a permanent USB device (32 GB flash USB with music) in #1, my Android phone in #2 which also provides Pandora (if I want) and a SiriusXM subscription.
AND... I have the upgraded Herman / Kardon system (19 Speakers or so).

I hear the music that comes from #1 (USB), Pandora shows (via Android Auto), and I think I hear SiriusXM all at the same time.
This has happened on 3 different occasions. (I only have 1025 miles driven.)
I have to disconnect my phone from #2, Turn on FM Radio and afterwards chose again # 1 USB, to get the right output.

Does anyone else have this issue?
Am I doing something wrong? (Settings, USB #1 should be removed etc.)

I don’t mean to be that guy, but there’s another in depth thread on this.

Tried to copy & paste but it doesn’t seem to be working, just search gremlins and it’ll come up
